By booking an appointment you agree to be bound by these Terms and Conditions.
All personal information you provide will be handled by Toni Fuqua operating as Toni Fuqua Makeup Artistry, LLC with responsibility and company liability. Your personal information will only be used to conduct business with you and will not be shared with other third parties unless given explicit permission. You always have the right to request Toni Fuqua to delete or correct your personal information.
Events outside Toni Fuqua’s control, which are not reasonably foreseeable, shall be considered force majeure. This means that Toni Fuqua Makeup Artistry is released from her obligations to fulfill contractual agreements. Example of such events are critical injury, death, fire or flood, or natural disasters. The force majeure also includes government decisions that affect the market negatively and products, for example, restrictions, warnings, ban, etc.
For Non-Bridal booking, the final balance is due 72 hours prior to services as one payment via PayPal. If final payment is not received 72 hours prior to services, the service will be canceled. For Bridal booking, the final balance is due (2) week before the wedding date as one payment. If final payment is not received (2) week prior to the wedding date the services will be canceled. NO PAYMENT will be accepted the DAY OF THE WEDDING. The person responsible for the balance of payment is either the Client who has signed the service contract or the person he/she has indicated on the booking agreement. Acceptable form of payment is debit/major credit cards via PayPal. All credit card transactions conducted by Toni Fuqua Makeup Artistry are handled and transacted through third party dedicated gateways. Credit card information is not stored and all card information is handled over SSL encryption. Note: PayPal payments are subject to a processing fee (2.9% plus $0.30) for each transaction.
SERVICE LOCATION AND REQUIREMENTS
Location of services for the event will be at the discretion of the client, however there are certain requirements Toni Fuqua will need to complete the makeup applications. Preferably, a “set up” table/work area will need to be made available at the stated location.
All brushes and makeup products are sanitized before and after all makeup applications. Any skin conditions or allergies should be reported to Toni Fuqua prior to application. Clients with known sensitive skin conditions are recommended to have the service provider perform a sample test on skin to test reaction. By booking an appointment, you agree to release Toni Fuqua Makeup Artistry from liability for any skin complications due to allergic reactions.
EARLY CALL TIMES
A $75 fee will be charged for booked appointments between the hours of 5:00am and 8:00 am. This is an additional fee and due at the time of booking your appointment.
A mileage fee will be charged for locations beyond the thirty (30) mile radius of Glen Burnie, MD (21060). The amount will be determined at time of inquiry. Travel fee is assessed for roundtrip travel. Travel fee is determined based on the current IRS Standard Mileage Rate, which is 53 cents per mile as of Jan.1, 2018.
Where parking, valet or toll fees may be incurred, the pre-determined amount will be included with the final bill and due (2) week before the event.
AIRFARE AND ACCOMMODATION
All costs for travel are to be paid by the client. Costs may include but are not limited to airfare, hotel (2 or 3 nights depending on destination), transportation to and from the airport in the departing and destination city, parking in the departing and destination city, per diem for meals if not all-inclusive, service incidentals, and all taxes.
A $25 late fee will be charged for every 30 minutes a Client is late for the scheduled appointment, or if scheduled services exceed the allotted time because of client delays.
REFUND AND CANCELLATION POLICY
A 50% deposit is required to confirm all dates/appointments. Deposits are NONREFUNDABLE and NON-TRANSFERRABLE. Bridal services require deposit and receipt of signed contract to secure a wedding date. Cancellations require a 24 hour notice. Missed appointments or bookings canceled without proper notice will result in a forfeiture of your deposit.
